What to Expect
- If you did not fill out the “New Patient Forms” below, please arrive 15 minutes early to fill out the forms.
- Please have your insurance information ready before arriving
- One of our great staff members will take you to the back in order to begin your appointment
- During the appointment, you will receive: necessary x-rays, an oral cancer screen, teeth photos, and we will check your gums to be sure they are healthy.
- To finish your first visit, you will have a personalized consult with Dr. Gardner. He will check your oral health, answer questions, resolve concerns, and help you create a treatment plan to reach your #smilegoals!

To make your visit more enjoyable and stress-free, please print and fill out BOTH the New Patient and the Financial Policy forms below. Don’t forget to bring both of them to your appointment!
